Create a word document template add fields/tags with information retrieved from database

All i need to create a word document template add fields/tags in places where values need to be inserted. The template will require tables and charts as well.

I am new to Open XML, I have gone through WDG with my understanding open office xml sdk reflect the document template and generate document output based on list value or XSD.

All i need is to generate the Word Template with Fields/tags, and need to get fields/tags names from Database.

please suggest some ideas/example regarding the same.
